The Orioles lost a game and another starting pitcher in Miami on Thursday night.

Left-hander Cade Povich exited in the fourth inning of the eventual 4-3 loss to the Marlins with what the club initially called left forearm discomfort, which makes him the fourth Baltimore starting pitcher to be sidelined with a health concern since the start of the 2026 season. Manager Craig Albernaz said Povich will undergo an MRI on Friday with the Orioles returning home after winning two of three games from Miami to conclude a disappointing 2-5 road trip.

Povich threw only 43 pitches and allowed three earned runs in three innings before reliever Yennier Cano replaced him to begin the bottom of the fourth.

“He said he felt it in the second, and then it really tightened up on him in the third,” Albernaz told reporters after Thursday’s defeat. “It’s on the outside of his elbow, so we’ll get some imaging and kind of see where we’re at.”

While the Orioles are expecting Opening Day starter Trevor Rogers to return as early as Monday after a case of the flu landed him on the 15-day injured list, Povich is now dealing with the latest ailment for a struggling staff that entered Thursday ranking 26th in the majors in starter ERA (4.97) and 25th in bullpen ERA (4.82). Baltimore lost veteran starter Zach Eflin to season-ending Tommy John surgery after he made only one start, and right-hander Dean Kremer has been on the IL since April 23 after straining his squad while working out between starts. The Orioles are also currently without closer Ryan Helsley, who is recovering from right elbow inflammation.

Povich has pitched to a 5.12 ERA in 19 1/3 innings this season.
